Mental health services can offer a plethora of benefits that touch every aspect of your life, from your emotional and psychological state to your relationships, self-improvement efforts, and even your physical health.
Referrals are only required for our Psychiatric Rehabilitation Program (PRP). Please use the form below to submit your referral. If you have not received a referral, we can connect you with a therapist to begin the referral process.

After we receive your form, a Dream Works staff member will reach out to schedule a screening meeting in order to assess and to determine the program’s ability to meet your needs.

Dream Works Health and Wellness aftercare support was remarkable. Recognizing that recovery is an ongoing process, the staff provided me with the necessary tools and resources to navigate life outside Dream Works.
They helped me create a solid relapse prevention plan, connected me with local support groups, and encouraged me to continue therapy on an outpatient basis. Knowing that I had a safety net to fall back on gave me the confidence to face the challenges that awaited me.
Today, I am proud to say that I am a different person—a person who is free from the shackles of addiction. This program saved my life, but more than that, it gave me a new lease on life. It taught me the value of self-love, resilience, and the limitless potential that lies within each of us.
